Winter Sports

Athletes must have a completed profile on the Summit Lakes Privit site if they plan on trying out or participating in athletics in the 2020-2021 school year.  Parents/Guardians will need to provide all information and sign all MSHSAA (Missouri State High School Athletic Association) documents through Privit.  Directions to create and access your account as well as copies of the physical examination and pre-participation forms are located on our website. Please Note: If you have used Privit before at the high school level, you will NOT need to create a new account; however, you will need to make sure you log out of the high school link and use the link specific to Summit Lakes to create your athlete’s profile.

Very importantly, your athlete will not be able to participate in 2020-2021 athletics (tryouts/practice) until all 6 steps are complete – please make sure you mark ALL athletic teams your student plans on participating in, even if they haven’t made the team yet.  Also, please make sure the copy of the physical you upload is legible, and the photo/scan is clear.  Your athlete’s Privit profile has to be reviewed by our office, which takes a few days, so please complete everything as soon as possible, so your student does not have to miss any time with his/her sport.  NOTE: Your athlete MUST have insurance coverage, and the carrier and policy number must be listed.  Also, please double check your Emergency Contacts information in the Personal Details section.

NOTE: The doctor’s Physical Exam Form will be good for two years from the date of the exam, unless otherwise indicated by the doctor.  The MSHSAA Pre-Participation Documentation, which contains the Health History, Student Agreement, Insurance, and Concussion Information, will be filled out electronically yearly in Privit.  If you have any questions, please email or call Stacy Patton – – 986-1384.

Please double-check your athlete’s Privit Account to make sure he/she has been cleared to participate.  You will see two green bars if your student’s information is complete, and he/she has been approved on our end.

Athletes who are not cleared by the afternoon of October 23rd will be able to attend but not tryout or participate until the process is completed.  If Privit information is submitted after October 23rd, we cannot guarantee it will be approved on the same day.  Athletes must have 14 practices before they are eligible to participate in a contest.

If your student-athlete has tested positive for COVID-19 after the season has begun, the athlete will need to have the MSHSAA Return to Play form filled out by a physician and returned to the Activities Office before he/she can participate.  The form will be filed in our health office.

Students who are coming from home for practice will not be allowed to enter the building until 2:30 pm.  Please do not drop off before then.  We want to minimize athletes in groups when possible.

Students participating in any middle or high school Missouri State High School Activities Association (MSHSAA) sponsored sport or activity are required to pay a yearly participation fee to help offset the costs associated with providing these opportunities.  At the middle school level, the participation fee per student is $25.00.  This fee must be paid and processed (may take a few days) prior to the first contest.  This fee can be paid online via the PowerSchool portal or by bringing cash/check (made payable to SLMS) to the office.
